We've been informed that in several Locals in District 4, Avaya members are being asked to volunteer to:

(1) Fill out a Background Screening Authorization/Consent Form;

(2) Fill out a Background Screening Questionnaire (3 pages);

(3) Schedule a drug screening.

There are three things we need you to do:

(1) Notify ALL your Avaya members NOT TO VOLUNTEER to fill out this form or participate in any drug screening.

(2) Let this office know if members in your Local have been approached to fill them out or if you, as Union leaders, were approached by Management about this.

(3) Let this office know if ANY members were pressured in any way to fill them out. If the member is given a work directive to fill them out, contact this office immediately.

We will keep you informed as we investigate and take action around this issue.
